Are eggshells appearing around and its levels are low? It may have reincarnated when you weren't looking.

Did you bother to stay in the same garden your chao is in?
Didn't you even constantly check its age throughout the time to make sure it's maturing properly...?

By the way when your chao is 5 he should die and if you treated it right he should come back to live in an egg.

How long do you play each day? A chao year is about 3 to 4 hours. You may want to leave the game on overnight.
